Residents of Stockton-on-Tees can be referred by their GP’s, Practice Nurses, Physiotherapists or other Health Professionals to either the Lite 4 Life: Weight Management Referral Programme or the Active Health: Exercise Referral Programme. Both programmes aim to improve aspects of your lifestyle; the person who refers you will be able to give advice on which is suitable for you.
Lite 4 Life: Weight Management Referral Programme
Lite 4 Life is designed specifically for people who are overweight or obese and want to lose weight through a change in their diet and physical activity levels. If your body mass index is between 25 – 50 and a medical professional deems that your are suitable to be referred you can access the following course:
Course cost: Free of charge
Course length: 10 weeks
Course duration: 1 hour per week
Course venues: Billingham Forum, Stockton Splash, Thornaby Pavilion
Course structure: The programme is a group session and is one hour a week that runs over ten weeks. The session consists of individual weigh-ins and weight management advice, covering different topics each week.

Active Health: Exercise Referral Programme
Active Health is designed for people who have a medical condition (not directly related to being overweight or obese) which can be improved by increasing their physical activity levels.
Course cost: £2.30 for consultations and each exercise session
£6.70 for a gym induction (if required)
Course length: 12 weeks
Course venues: Billingham Forum, Northfield Sportsdrome, Stockton Splash, Thornaby Pavilion
Course activities: Gym based exercise, low impact exercise classes or swimming

What Happens Next
A medical professional will send your referral form to the Health Team in Tees Active. Upon receiving the referral form a member of the team will contact you to arrange a consultation.
